This is Survey Form #2 intended to be completed by persons who have already received the free sample of Herbal MSM, or obtained some of this Herbal MSM and wish to contribute to the public information about this new product.
There is an offer of a free sample to those who complete another survey form, located here. Depending on the date you might request a free sample, there may be some of the original batch still available -- give it a try if you have not already done so.
In any event, if you have purchased Vibrant Life Ayurvedic Herbal MSM, or received the free sample, and not already completed THIS survey form, you are cordially invited to do so.
If you did NOT respond to the first survey form, and did NOT receive the free sample, you can still complete the FIRST survey form, here, and thus become eligible for completing THIS SECOND FORM, and thereby earning a 50% discount on a future purchase as described below.
If we have BOTH survey forms from you, complete, and you have received a free sample of Herbal MSM from us, we will mark your record in our computer that you are eligible for a special one-time discount on one future purchase of Herbal MSM. The Herbal MSM comes in two packages sizes -- one is a bottle of 300 capsules and the other is a jar of 32 ounces of loose powder. Both of these forms of package have a discount schedule. You pay the "regular" price for one container, and there is a 25% normal discount when you get four of the same container.
This one-time, special, discount when you have completed both survey forms, is for 50% instead of 25% on either form of Herbal MSM, whether you use this special discount on one bottle/jar or up to four.
Presumably you would not want to take advantage of this discount if you found this product to be unsatisfactory in your first use. Our experience has been that some number of people do not like the taste of the fresh herbal extraction with the MSM.
The formal launch of Herbal MSM was/is scheduled to be June 25, 2003. However, the samples were available at an earlier date and we will be honoring sample requests before that date. The Free Sample Offer will be announced, formally, by Karl Loren's newsletter published on June 25, 2003, but available before then to those who find these pages. We expect to have more samples available.

This survey is extensive. Your answers will help others decide whether or not to use Herbal MSM, what to expect and how to use it. Note that many of the drugs often used for pain relief are described in detail in a study on pain killers.
